Vision Screening Program – Year 6 Students
OPSM Maroochydore have kindly selected Kuluin to be a part of their complimentary Vision Screening Program.  The program offers a FREE Vision Screening to Year 6 students at Kuluin.
A vision screening is an efficient method that may identify children with visual impairments or eye conditions*.  The main goal of vision screening is to identify children who have or are at risk of developing any vision or health issues with their eyes.
OPSM’s qualified Optical Dispensers perform the screenings during school time at school.  Each child takes approximately 5-10 minutes to complete the non-invasive test.  After the screening has been performed – a screening report is sent home to parents/guardians**. A record of the result is also kept by the school. 
At this stage the dates for assessments will be Wednesday 2 and Thursday 3 November with follow-up days the next week, depending on the number of responses.
Attached to this letter is a parental permission/eye health history form.  Completion of this form gives OPSM permission to screen your child’s vision at our school.
If you would like your child to have a complimentary vision screening – please complete and return the attached form to your child’s classroom teacher by Monday 24 October.
Please note that notes returned after this date cannot be accepted.
